You will be sent a link to complete the online application and pay the $150 non-refundable enrollment fee.
Required Documents
Gather and submit the following documents:
- Copy of certified birth certificate
- Immunization records on Florida Department of Health form DH-680
- Current physical examination on Florida Department of Health form DH-3040
- Two proofs of residence (acceptable documents)
- Proof of Residency
- TWO proofs of residency are required. Each proof MUST include name and current physical address. Post office boxes, private mailbox addresses or commercial addresses are insufficient.
- Apartment rent receipt or home lease agreement, mortgage document, or property tax record.
- Current utility bill (electric, gas, water), cable, or home phone bill
- Current Voter registration document, State of Florida identification card
- Proof of current government benefits (disability, Medicare, SNAP food stamps, Department of Children and Families (DCF) correspondence)
- Current Florida driver’s license, automobile registration, automobile insurance
- Withdrawal form from the school student previously attended
- Legal Documents: Guardianship or any family law forms that need to be upheld by the school
- Health Information Form
- IEP or 504 Plan if applicable
