Admissions at Driven With A Purpose Academy Inc.

Please follow the steps below to enroll your child at Driven With A Purpose Academy Inc. Completing each requirement by the stated deadlines will ensure your child’s seat is reserved for the upcoming academic year. If you have any questions, please contact us at dwapacademy@gmail.com.

Step 1: Submit an Initial Inquiry Form

Start the admissions process by submitting our Inquiry Form. After we receive your submission, we’ll send you comprehensive information about the academy and outline the next steps.

Step 2: Family Consultation

Once you have reviewed our materials, you will schedule a consultation with our admissions team. This meeting will allow us to learn about your family and answer any initial questions.

Step 3: Application Submission

You will receive a secure link to complete our online application. To process your application, a $150 non-refundable enrollment fee is required at this stage.

Step 4: Submission of Required Documentation

Please provide the following documents:

  • Certified copy of your child’s birth certificate
  • Immunization record (Florida Department of Health form DH-680)
  • Current physical examination (Florida Department of Health form DH-3040)
  • Two proofs of residency (accepted forms required)
  • Withdrawal from your child’s previous school
  • Any applicable legal documents (such as guardianship or family law forms)
  • Completed Health Information Form
  • IEP or 504 Plan, if applicable

Additional forms may be requested annually.

Relationships are central to our approach. Before enrollment, your child will be invited for a student meeting. This gives us insight into their interests, strengths, learning preferences, and social interactions, enabling us to personalize their academic experience.

Step 6: Placement Assessment

To ensure appropriate academic placement, every student completes placement assessments in reading and math. These diagnostic tests are administered free of charge during the student meeting.

Step 7: Parent Conference

The admissions process concludes with a parent conference. We will review your child’s assessment results, discuss strategies for home support, and answer any questions you may have before your child’s first day at Driven With A Purpose Academy Inc.
